What it is
An epidermoid or sebaceous cyst is a sac beneath the skin filled with keratin — the same protein that makes up the surface of your skin. They feel like a smooth, firm lump that moves with the skin, often with a small central punctum. Pilar cysts are the same kind of lesion arising from hair follicles, most often on the scalp, and are treated and priced identically. All are benign, but they grow slowly, they can become inflamed or infected, and once emptied they refill unless the sac wall is removed completely.
When treatment is appropriate
Where a cyst is recurrently inflamed or infected, is growing, is painful, catches or is repeatedly traumatised, or is interfering with function. Cysts that are actively infected are treated medically first and excised once settled — we will not excise into infection.
Why removal, not draining
Squeezing or lancing a cyst empties it temporarily. Because the sac remains, it refills. Surgical excision removes the sac wall intact, which is what stops it recurring — and is why the procedure takes longer and costs more than simple drainage.

How it works
Local anaesthetic, an incision over the cyst, careful dissection of the sac wall so it comes out whole, then closure with sutures. Suture removal 7–14 days later, included. Scalp cysts may require a small area of hair to be parted or trimmed at the site.

Frequently asked questions
Will it come back?
Not if the sac is removed intact, which is the aim of excision.
Why can't you just drain it?
Draining empties the cyst but leaves the sac wall behind, so it refills. Excision removes the sac and is what stops recurrence.
What if it's infected?
An actively infected cyst is treated medically first and excised once it has settled.
Is a cyst on my scalp treated differently?
Pilar cysts are treated in the same way and at the same price.
